W.O.R.T.H. Center Aftercare Program
 
  • Worth Center currently offers aftercare in the following seven of nine counties serviced: Auglaize, Hardin, Mercer, Paulding, Putnam, Shelby and Van Wert Counties. (Allen and Hancock Counties have their own aftercare programs in place)
  • This grant is currently being funded by the Probation Improvement and Incentive Grant (PIG Grant). Mercer County Court of Common Pleas/Adult Probation have coordinated efforts with our facility in hopes that providing contract services it would allow probation departments to provided skilled professionals and programming to the probationers to address post release issues.
  • Currently all identified seven counties provide office space at their court houses, with the exception of Paulding which offers space at their law enforcement office. There are currently three aftercare workers, one handles Auglaize and Shelby Counties, one handles Van Wert and Mercer Counties, and one handles Hardin, Paulding and Putnam Counties.
  • Auglaize and Shelby Counties services include the following: Client meets with Aftercare worker prior to meeting with Probation Officer. Each aftercare worker is trained in cognitive interventions specializing in TFAC and University of Cincinnati’s Substance Abuse Program. Aftercare workers assist clients in obtaining public assistance if needed and likewise also assist in finding employment. Aftercare worker assists in continuing to practice CBT Interventions in real life settings. Workers are equipped with cars, phones, laptops, food cards, Walmart cards, and gas cards to assist in immediate barriers. Auglaize County Common Pleas Court has a Medically Assisted Program (MAT) in place and this is a new program that the local Judge is handling along with local service providers. Currently Shelby County is considering a drug court however it is very preliminary at this point.
  • Mercer and Van Wert Counties Services provided are the following: Client meets with Aftercare worker prior to meeting with Probation Officer. Each aftercare worker is trained in cognitive interventions specializing in TFAC and University of Cincinnati’s Substance Abuse Program. Aftercare workers assist clients in obtaining public assistance if needed and likewise also assist in finding employment. Aftercare worker assists in continuing to practice CBT Interventions in real life settings. Workers are equipped with cars, phones, laptops, food cards, Walmart cards, and gas cards to assist in immediate barriers.   Van Wert County Common Pleas is currently developing a drug court and will soon have their first participant, our clients will be able to benefit from this added support once in place. This court is also considering a MAT program and working towards development. Mercer County currently has a drug court which is involved with MAT and has several current participants.
  • Hardin, Putnam, and Paulding Counties include the following for clients: Client meets with Aftercare worker prior to meeting with Probation Officer. Each aftercare worker is trained in cognitive interventions specializing in TFAC and University of Cincinnati’s Substance Abuse Program. Aftercare workers assist clients in obtaining public assistance if needed and likewise also assist in finding employment. Aftercare worker assists in continuing to practice CBT Interventions in real life settings. Workers are equipped with cars, phones, laptops, food cards, Walmart cards, and gas cards to assist in immediate barriers. 
  • Putnam and Paulding Counties are working towards coordinating efforts in their county to hopefully move towards a MAT program. Hardin County currently has a Recovery Court that meets weekly, counseling agency, probation, children services and our worker all provide support services collectively. The individuals we have in our program from Hardin County receive Naltrexone while in our program and the local counseling agency follows up with the first injection of Vivitrol prior to their release. ​
